Admission Form

Questions:

Answers:

  1. When will I receive my admission form for the 2019 HKDSE?

Admission forms for Category A written and speaking examinations will be distributed to candidates on 13 February 2019. If you are going to sit any Category A practical examinations (i.e. Physical Education Paper 3 or Music Paper 2 or Paper 4B), you should have received an admission form for practical examinations in mid-January 2019.

School candidates will receive their admission form via their school whereas private candidates will receive their admission form by post.

  2. If I do not receive my admission form at the times mentioned in A1 above, what should I do?

Candidates who do not receive their admission form for written and speaking examinations by 25 February 2019 should come to the HKEAA Southorn Centre office in person (Address: 12/F, Southorn Centre, 130 Hennessy Road, Wan Chai, Hong Kong) to apply for a re-issue of their admission form. They should bring their Candidate Entry Information Checklist and identification document. Candidates who request a re-issue of their admission form for practical examinations or those who are holders of the juvenile HKID card should submit a 3cm x 4cm photograph. School candidates are required to produce a school letter. There is no charge for this service if you make the request before the day you sit for the first examination session.
